ABC 04-04 ANTIBIOTICS FOR ULCERS (The Science Show: 31/3/2001)
In 1983 Barry Marshall announced that stomach ulcers were caused not by stress but by the bacterium Helicobacter, and therefore could be treated with antibiotics. He developed a series of tests to diagnose infections and is now working on ways to eliminate the Helicobacter bacterium altogether. Last week Dr Barry Marshall was recognised with a Clunies Ross Award for his discovery.

Wurtzbourg/Berlin/USA. Des travaux menes aux USA, dans un institut Max Planck de Berlin ainsi qu'a l'universite de Wurtzbourg, ont mis en lumiere une etonnante propriete d'Helicobacter pylori, une bacterie qui infecte plus de la
moitie de la population mondiale. Cet unicellulaire est l'un des rares a pouvoir coloniser la muqueuse stomacale, ou il declenche une inflammation qui peut s'etendre sur plusieurs annees, voire un ulcere. Le traitement classique a base d'antibiotiques n'est pas sans rencontrer quelques difficultes. Pourquoi?
Les resultats de l'etude, conduite par les microbiologistes Sebastian Suerbaum et Christian Kraft, plaident pour une cause genetique. A l'aide d'un modele mathematique, les chercheurs ont decouvert que la bacterie est capable de muter soixante fois par an. Grace a cela, elle est en mesure de modifier la moitie de son patrimoine genetique en l'espace de 41 ans, un delai extraordinairement court, meme a l'echelle de l'etrange petit monde des bacteries.
Ces mutations permettent a Helicobacter pylori de resister au mieux aux antibiotiques. La science devra tenir compte des resultats de ces travaux, si elle veut mettre au point des traitements efficaces sur le long terme.

La bactérie responsable de la majorité des ulcères de l'estomac est Helicobacter pylori.
Il est encore des médecins (y compris formateurs de médecins) qui ne le savent pas.
Pour réaliser l'antibiogramme il faut isoler la bactérie : cette opération n'est pas simple vu qu'il va falloir aller la chercher dans l'estomac du patient. Une fois isolée il faut la cultiver.
Il est beaucoup plus simple, une fois le diagnostic posé, de faire un premier essai d'antibiothérapie (qui fonctionne dans 90 % des cas), et de changer ensuite d'antibiotique en cas d'échec. Plus simple pour tous, moins traumatisant, moins coûteux.
Cette antibiothérapie probabiliste est la règle pour de nombreuses infections.

/H. pylori/ vit exclusivement dans l'estomac humain et est le seul organisme connu pouvant survivre dans un environnement aussi acide. Elle possède une forme hélicoïdale (d'où le nom de « /Helicobacter/ ») et peut se visser dans la paroi stomacale afin de la coloniser 80% des ulcères gastroduodénaux, mais en Europe nous avons de la chance: des médecins et la sécurité sociale, mais comment font les autres?

Dit autrement les ulcères cela s'exprime avec le même risque dans toutes les cultures ?

avant d'être aussi stricte, il faudrait peut être bénéficier d'autres informations.

Cette étude semble faites in vitro et la différence peut être remarquable entre l'in vivo et l'in vitro.

Ma question : quelle quantité de wasabi doit on consommer pour avoir les effets constatés in vitro.

De plus l'effet bactéricide doit être temps dépendant. Dois je manger du wasabi 1 fois, 2 fois par jour ou en mastiquer toute la journée.

En conclusion, si pour avoir le même effet qu'une association d'antibiotique je dois mastiquer en permanence du wasabi et en consommer au moins un kilo par jour ... j'ai vite fait le choix.

Avez vous déjà gouté du wasabi ?

Je pense plutôt que cette étude peut tendre à expliquer une éventuelle différence de prévalence d'ulcère infecté par helicobacter entre les japonais et les autres. (si différence il y a) et apporter un début de piste pour le développement d'un nouveau médicament. D'ailleur ils le disent eux même, ils n'ont pas encore trouvé toutes les molécules bactéricides contenues dans le wasabi.

quid de la prévalence de ces affections en fonctions des pays et des régimes alimentaires ?

Pour votre information, il semble, à ce stade, impossible, en France, de disposer de la répartition du taux de cancer, par exemple, dans le département des Bouches du Rhône et je suis sûr, désormais, que ceci a pour raison, des raisons bien peu honorables.

Il semble que le wasabi soit un très sérieux sujet d'études, comme le sont d'ailleurs la plupart des aromatiques (même des peu connues comme Bunium persicum nous offre une abondante littérature), revisitées progressivement par les scientifiques, en quête et de nouvelles applications et de validations (ou d'invalidations) des trucs de grand-mère.

Pour macher du wasabi toute la journée, hormis l'agréable feu qu'il dégage, il faut surtout disposer d'un épicier hors-paire, vu la rareté de cette brassicassée.

vous avez raison, et cela a même fait dire à certains que le nombre de gastro-entérologues devait baisser car la gestion par les antibiotiques a supprimé x motifs de consultation.

Mais Il n'empêche que le cout de gestion a été important et que si cette épidémie en France, au moins, avait pu être évitée par un équilibre alimentaire plus approprié et ou des conseils plus opportuns, cela aurait, de mon point de vue, était beaucoup mieux.

J'ai choisi de parler de ce sujet surtout pour montrer comment se constitue, dans une société donnée, la démarche scientifique. Elle est surtout, de mon point de vue, le reflet de la guerre de clans et ou de corps, ce qui est désolant dans une société qui s'est engagée à promouvoir la paix.

Heureusement que désormais on peut disposer de plusieurs regards...
